Through June of 2024 BSSP's free Health Screenings (formerly Health Risk Assessments) are offered through a trial partnership with Quest Diagnostics.

These are available to all retirees and dependents over the age of 18 who are covered under a SISC+BSSP non-Medicare Retiree Medical Plan.   At this time those covered under the CompanionCare Medicare Supplement plan are not eligible.

Total cholesterol, HDL, blood pressure, pulse, blood glucose, body mass index, and other key biometrics will be checked during your appointment at a Quest Patience Service Center.  The screening will take about 15 minutes, is confidential, and is protected by HIPAA privacy laws.

Create your account and make an appointment by following the instructions below:

• Visit my.questforhealth.com 
• Enter your Registration Key: Butte2024 and click the Register Now button
• Provide information as prompted to complete the registration process
• In the Wellness Screening section, you may select the most Quest Patient Service Center for you. 
• Select your preferred date from the options listed in bold, then the time you'd like.  
• Click the Confirm button. Your appointment is not scheduled until you click Confirm.

Once your Health Screening appointment is completed, you will receive a $25 Amazon award via email within 30 days. Once claimed, Amazon Gift Codes never expire and can be used for anything available on Amazon.com.
